In the event that
you are experiencing problems with an Illumination Technologies product we will
provide you with a Return Materials Authorization (RMA) number so that we can
get the unit in house and evaluate it. Additionally if you have an item or items
on an evaluation basis we will provide you with an RMA number to return the equipment.
Please fill in the form below as completely as possible. Indicate in as much
detail as you can on the issues your are having with the equipment, or simply
indicate that it is an evaluation return. When you receive your RMA number pack
the equipment, in the original box if possible, and write the number on the
outside of the box, and return it to:
